Policies and Forms

I’ve found it helpful to use the following forms in my practice. The forms either help clarify what clients can expect from their sessions with me and/or ask for their consent for services.

After we've scheduled our first Brennan Healing Science session, I'll ask you to review and sign the Information and Consent Form along with the Policies Form. I may also ask you to complete the Client Intake Form. The forms are described below. Click on a form name to review it.

For Brennan Healing Science Services

BHS Information and Consent Form

This form describes my training and what clients can expect during a session as well as securing consent.

BHS Policies Form

This form lays out my fees and accepted methods of payment. It also defines my policies for cancellation, lateness and returned checks.

BHS Client Intake Form

This form allows me to collect important information about my clients so that I can individually tailor their sessions to be as effective as possible. All information on this form will be held in the strictest confidence.

For Massage Therapy

Massage Client Intake and Consent Form

Prior to starting a massage session, clients will be asked to complete the Intake & Consent Form. This form gives me permission to work with clients and draws my attention to any possible contraindications, which will be discussed prior to the massage.
